NTST have had a request from a school nurse for guidance and examples of health action plans. Her email and our response is below. If you have further examples of guidance or examples, please let me know and I will pass them on to her.

From school nurse:
I work has a school nurse within a special school for young people aged 11-19 with severe learning disabilities and complex health needs. I am currently looking at setting up health action plans to aid transition. I am working closely with various other professionals, do you have any guidance or experience in setting up such plans.

I recommend you look at our Transition Guide for All Services that we produced for DCSF in 2007, as it briefly covers health action plans. Hi we are in the porcess of developing Health Action plans in collaboration with paediatric and adult services. We have found that HAP can be used for two main pruposes and ideally should be designed differently for both. The first one is to identify services and euipment sources and ensure their eligibility criteria are considered. The plan reflects health needs, services which will provide care with named individuals and who will be responsible for equipment. The second is to use the HAP as a tool for carers and young people to help them in preparing and planning for transition- the template produced by DOH in the Moving on well document is suited for this and were are going to start giving it to parents and pilots its use. Hello

We have started using both the Moving on Well and the Transitional Care Planning Pathway documentation. The Moving on Well documentaion is fairly comprehensive and I have found that you really need to take your time and it is important that each section is well evidenced to avoid any confusion later. We are also looking to develop an easy read version for young people with learning / communication difficulties. It is also vital that all partners are signed up, we are working across primary and secondary care trying to embed standards regarding the documentaion , but it is early days.
