We are a new Health Transition team based in the West Midlands set up to initially address the health needs of young people with physical difficulties in mainstream education.

Our trust has a well established LD team, but the children in mainstream were slipping through the net. We have devised our own assessment based on the government model in the ‘Transition: Moving on Well’ document, plus our own Health Action Plan/Passport paperwork. We are currently trialling these with young people aged 14 to 18 with physical difficulties, and will audit their use after 6 months. We have also organised ‘Staying positive’
workshops through the Expert Patient Programme to help the young people take responsibility for their own health care.

The plan is to obtain more funding so that we can extend the team to cater for ASD, ADHD, mild LD etc. in the future.

Is anyone else doing the same sort of thing, as sharing of ideas would be very useful to evaluate our processes?

Kathie,

I am currently working on evaluation of a transition process for young people who attend the diabetes service within our Trust.

Our intention is to develop this work further and we have secured funding to begin a qualitative study with young people and families where the young person has a physical disability to investgate what sort of transition services (health) would be useful for them. We are currently writing a general policy for our Trust but want the views of young people and their families to be central. Our LD services are well set up but we are aware that young people come out of paediatrics to a complexity of adult services (some not in our Trust). We are applying for NHIR funding to extend the work across other centres (Trusts) for young people with Autism and ADHD.
I would be very interested to share info and ideas.
