personalisation that includes person centred planning, work

TransMap Case
Studies
These case studies are taken from the
Council for Disabled Children’s TransMap project in 2008/09. This
project set out to identify areas of effective practice in
transition and to provide evidence of current practice and
developments.

Advocacy Project
