Case studies

The following case studies show examples of good practice that local areas have been developing through the Transition Support Programme.

 

City of York Council (Post 16 opportunities)
This case study describes how City of York Council is supporting disabled young people through its York Independent Living and Travel Skills Initiative (YILTS).

City of York Council (Post 16 opportunities)
This case study describes how City of York Council set up an Opportunities Fair to provide better information about what disabled young peope could do after leaving school.

Dudley Metropolitan Borough Council (multi-agency working)
This case study describes how Dudley Metropolitan Borough Council is using multi-agency working to improve local services and reduce out of borough placements.

Hertfordshire County Council (joint assessments)
This case study describes how Hertfordshire is using the Learning for Living and Work Framework as the basis for joint planning.

Lincolnshire County Council (Participation)
The case study describes how the creation of a shadow transition board has ensured that the voice of disabled young people is central to and informs the developing work of the transition service in Lincolnshire.

Nottingham City Council (multi-agency working)
This case study describes how Nottingham City Council has set up a multi-agency transition team with Connexions at the hub of the transition process, overseen by a multi-agency transitions steering group.

Oldham Borough Council (employment)
This case study looks at the development of employment and work experience opportunities for disabled young people in Oldham.

Oldham Borough Council (housing)
This case study describes two different schemes which are helping disabled young people in Oldham to achieve their goals of having their own home, accessing mainstream facilities and living in the community with varying levels of support.

Richmond upon Thames (personalisation)
This case study looks at how a group of young college leavers with learning disabilities in Richmond upon Thames have piloted an innovative way of getting more from their direct payment care package.

Richmond upon Thames (participation)
This case study describes how a voluntary organisation in Richmond upon Thames is helping young disabled people develop training on disability awareness for a range of organisations in the borough.

Suffolk County Council (person centred planning)
This case study describes how Suffolk is embedding person centred approaches in all special schools with the long term goal that person centred planning will be universal for disabled children in the authority.

Thurrock Council (participation)
This case study describes how Thurrock Council have used drama workshops to support students with complex needs learn about transition and how they can make choices about their lives.

Thurrock Council (personalisation)
This case study shows how Beacon Hill School in Thurrock is using a whole school approach to personalisation that includes person centred planning, work experience, personalised curriculums and support into mainstream college settings.

West Berkshire Council (multi agency working and joint assessments)
This case study describes the progress that West Berkshire Council has made in developing their transition services over the past 3 years.

Emerging good practice

TransMap Case Studies
These case studies are taken from the Council for Disabled Children’s TransMap project in 2008/09. This project set out to identify areas of effective practice in transition and to provide evidence of current practice and developments.

Advocacy Project for Transition
North Tyneside, with funding from the Transition Support Programme, have commissioned 2 years of advocacy to support young disabled people with their Transition Plans.
