TransMap Case
Studies
These case studies are taken from the Council
for Disabled Children’s TransMap project in 2008/09.
This project set out to identify areas of
effective practice in transition and to provide evidence of current
practice and developments.
The local area case studies include
information on the following issues: Developing a strategic
transition protocol and pathway; post-16 options - education and
employment; transition reviews and person centred approaches; an
overview of statutory services; health services; Connexions and
IAG; providing information about the transition process;
leisure services; youth services and activities; housing and
transport.

Birmingham
Birmingham have a Transition Manager who is responsible for the
transition of all vulnerable young people, including disabled
children and young people with special educational needs.
Blackburn
College
How the Additional Learning Support Service supports learners with
additional needs.
Bradford
An overview of Bradford's support for young people, including the
development of a strategic transition protocol, a work experience
project and the use of person-centred approaches.
Brighton and
Hove
Brighton and Hove has a multi-agency Transition Forum that is
responsible for the overall development of transition
processes.
Leicester
City
Leicester College have been working with Adult Social Care Services
and Remploy on a 3-year programme, which enables young people who
attend the college to have work tasters throughout their college
life, to smooth the transition into employment after college.
MacIntyre
How the 'My Way' project supports disabled young people and their
families in the transition from MacIntyre schools into adult
services.
North East
Lincolnshire
North East Lincolnshire has provided ‘Getting Heard’ training for
young people with a disability to enable them to represent the
views of their peers, participate effectively in school council
meetings and at the Council for Young People.
Richmond
On Thursday evenings there is a Transition Club for disabled young
people, which is run in partnership with the youth services and
Crofters (a service for disabled children). They are also
looking at improving access to mainstream clubs.
Redbridge
Redbridge has a specialist Connexions adviser that works one day a
week within the Children with Disabilities Team. Her main role is
to advise disabled young people on financial and social issues.
Sunderland
City Equals is a group of disabled young people aged between 13 and
25 that work together to remove barriers to inclusion.
Sunfield
School
Sunfield School has developed a Transition Profile for young people
using a Person Centred Approach. The profile gives details
of support and care needs and includes a section
‘All About Me’ that has information about health needs,
communication, hopes and dreams for the future, keeping healthy and
safe, things I like doing and people that are important to me.
Surrey
Surrey has been running a Person-Centred Transition Reviews
Project. The vision of the project is that all young people with a
statement of special educational needs will have a person-centred
transition review.
West Gate
School
West Gate School have employed a Communication Teacher whose role
it is to develop communication passports for all young people with
communication needs.
