The National Transition Support Team is not resourced to give direct advice to professionals, families or disabled young people about individual cases.

The following organisations and websites provide advice, support or resources that professionals, families and disabled young people might find useful.

 

ACE  (Advisory Centre for Education)
The Advisory Centre for Education (ACE) is a national charity that provides independent advice for parents and carers of children aged 5-16 in state-funded education. ACE aims to provide the information, support and high-quality advice that parents need to help their children at school, particularly where there are problems. They offer free advice on many subjects like exclusion from school, bullying, special educational needs and school admission appeals.

C4EO - Centre for Excellence and Outcomes in Children and Young People's Services
C4EO's work on disability supports Directors of Children's Services and their Children's Trust partners by providing the knowledge, data and tailored support they may need to help improve outcomes for disabled children and their families.

Commissioning Support Programme
The Commissioning Support Programme will help Children’s Trusts plan, design and implement services more effectively. The Programme aims to help Children’s Trusts deliver better outcomes for children, young people and their families.

Run by a consortium of five partner organisations appointed by the Department for Children, Schools and Families and Department for Health.

The Family Fund
Provides grants to families caring for disabled young people aged up to 16 in England, Northern Ireland and Scotland and for young people aged up to 17 in Wales.

National Parent Partnership Network
Parent partnership services are statutory services that offer information, advice and support for parents of children and young people with special educational needs (SEN). All parent partnerships, wherever they are based, are at ‘arm’s length’ from the Local Authority and so are able to provide impartial advice and support to parents.

Transition Information Network (TIN)
TIN is an alliance of organisations and individuals who work together with a common aim: to improve the experiences of disabled young people’s transition to adulthood. TIN is a source of information and good practice standards for disabled young people, families and professionals.
