The deadline for completing the year 1 SAQ has now passed. 147 local areas completed the SAQ by the deadline and every local area that submitted has been sent a copy of their completed SAQ. If you have not received a copy of your local area’s SAQ please email us on TSP@ncb.org.uk

The SAQ will be completed online. However a printable word version is available to use in meetings to help in developing answers for the SAQ and taking stock of local planning. If you are the nominated person in your area to complete the online SAQ, please email tsp@ncb.org.uk to receive the web link.

 

About the SAQ (Year 1 SAQ)

One of the first tasks of the national transition support team is to build on existing evidence and form a picture of what transition services in all local areas are currently delivering to disabled young people and their families.

Therefore, we have developed a Self Assessment Questionnaire (SAQ) for local authorities to complete in partnership with Primary Care Trusts, partner agencies, disabled young people and their families. All local authorities will be sent a letter telling them how they can access the SAQ.

The purpose of the SAQ is to identify and determine how well each local area is working towards or beyond minimum standards for transition.

The information received from the SAQ will help us to assess and identify the type and level of support each local area needs to improve their transition services. The national transition support team will then provide a report to the DCSF, with recommendations of what level of support and funding is needed in each local area.

The SAQ will be carried out each year of the Transition Support Programme and each local area will be assessed on current practice and evidence.

The SAQ is based on the key principles set out in both the DCSF’s A Transition Guide for All Services (2007) and the Department of Health’s Transition: Moving On Well guide (2008). These guides were designed to illustrate what minimum standards in transition support are. The aim of the Transition Support Programme is to look beyond minimum standards and into creating flexible, individualised, responsive and creative services to enable disabled young people to meet their full potential.

The deadline for completing the questionnaire is 30th January 2009.

We appreciate this timescale is short. This is to ensure we have information from each area to plan for year two grants and support allocations of funding.  More time will be given in future years.

 

Before you start

To help you gather the answers you need, we have created a word version of the questionnaire. Before you complete the online questionnaire, use the word document to collect your responses.

Please make sure everyone with responsibility for transition in your local authority and PCT is in agreement with the answers you will be submitting. Please also refer to the Guidance NotesFAQs and Longer Explanatory Notes before getting started.
